The Prognostic Significance of Number and Location of Positive Lymph Nodes in Rectal Cancer



Abstract
To evaluate the prognostic differences between the number and the location of positive lymph nodes in retal cancer, we retrospectively studied with 147 cases of curative resection for rectal from 1980 to 1989.
@ES the rusults were as follow;
@EN 1) The mean age was 54 years old(range 28~84), male to female ratio was (1.04 : 1). The common location were lower rectum(64.0%), upper rectum(21.8%), rectosigmoid(12.2%) and anal canal(2.0%) in that order. The A-P resection(88 cases) and low
anterior rection(44 cases) were most common operation.
2) We dissected total 2121 lymph nodes, 427 of them were positive. There was an average of 14.4 lymph nodes per case and 2.9 positive lymph nodes per case.
3) The five year survival rates for curative resected cases by Dukes' stage. Dukes' A were 100%, Dukes' B were 79.5% and Dukes' C were 46.5%, respectively.
4) The five year survival rates by the number of positive lymph node. We divided the patients into no(no positive node), Nl (1-3 positive nodes) and N2(4 or more positive nodes). The 5 year survival retes were 85%, 65% and 30%, respectively.
5) The survival rates by the location of positive lymph nodes. The two year survival rates were 76.1%(nl), 50.8%(n2), 26.7%(n3) respectively.
6) The relationship between the number of positive nodes and recurrence & metastasis.
The recurrence rates were 42.4%(N1) and 41.7%(N2).
The rates of metastasis were 27.3%(N1) and 27.8%(N2).
7) The relationship between the location of positive nodes and recurrence & metastasis.
The recurrence rates were 42.6%(n1), 40%(n2) and 40%(n3).
The rates of metastasis were 24.2(n1) 30%(n2) and 40%(n3).
